Australian Childhood Immunisation Records

Vaccination coverage rates for children entering school in WA are currently among the lowest in Australia and well below the national target of 90%.

Outbreaks of vaccine preventable diseases such as measles can have serious health consequences. To prevent this, coverage rates need to be 95% or higher.

The School Education Act of 1999 requires parents to provide their child's immunisation records when the child enrols in school.

Parents can request an Immunisation History Statement at any time by telephoning ACIR on 1800 653 809 (free call) or emailing acir@humanservices.gov.au.  Alternatively, parents can log into their MyGov App, click on Medicare and follow the links.  If parents can't access MyGov, they could either visit their local Department of Human Services Centre or Medicare Office.

IMPORTANT MESSAGE FOR ALL KINDY PARENTS  -  When your child moves from Kindy to Pre Primary a notification of the 4 year old vaccination needs to be presented to the school office so that this information can be added to your child's records.

PARENTS ARE ADVISED THAT IMMUNISATION DETAILS ARE REQUIRED AS PART OF THE ENROLMENT PROCESS AND THE ACIR IMMUNISATION HISTORY STATEMENT IS REQUIRED BEFORE THE CHILD COMMENCES SCHOOL.
